Application Procedure for Preschool  for SY 2021-2022 ONLY

 

1.Review the admission procedure and list of requirements; and download the LSCA       Admission forms for Preschool HERE

2.Scan (only) the following for email submission:

a.Application Form (completely filled out)

b.Disclaimer Form

c.Privacy - Consent Agreement Form

d.Student’s Medical Record (child’s immunization record)

e.One (1) latest I.D. picture, 2” x  2” size

f.Duly signed Final Report Card ( scan and email all pages) for Kindergarten Applicants    enrolled in the previous year

g.PSA copy of Birth Certificate

h.Baptismal Certificate or its equivalent for non-Catholic (optional for email submission)

 

3.Send to: admissions@lsca.edu.ph

   Please enter as Subject: Application for (preschool level) e.g. Application for Nursery 1

4.ASAO will notify you through text and email regarding status of your application and       instruction on how to send payment for the Application Fee of Php450.00

5.The scanned copy of the Acceptance Letter will be emailed to the successful applicant

a.Carefully read the contents of the Acceptance Letter

b.Tick the options provided in the Reply Slip before signing

c.Scan and send the signed Reply Slip to admissions@lsca.edu.ph

6.Confirmation upon receipt of your Reply Slip will be sent through text or email

7.You will be notified by our office on the Enrollment date of your child

*** Please note that your child is conditionally enrolled until all original requirements are submitted to LSCA Admission Office once we have resumed to normal operations
